Application to Graduate - Undergraduate Students
This must be turned into the Student Service Center the semester prior to the completion of your degree. Please read the instructions carefully as the information on this form is used when creating your diploma. Make sure to come into the Student Service Center to complete a Senior Credit Evaluation before your final semester to ensure that you will complete your degree on time.

Application to Graduate - Graduate Students
This must be turned into the Student Service Center the semester prior to the completion of your degree. Please read the instructions carefully as the information on this form is used when creating your diploma.

Course Withdrawal Form - Fall 2008
Students who wish to withdraw from a Fall 2008 course after September 19, 2008 through December 1, 2008 must complete this form and turn it in at the Student Service Center (course will be graded with a WP or WF).  Students may not withdraw from Fall 2008 courses after December 1, 2008.

Per Credit Tuition Application
Second semester seniors who need more than 11 credits but less than 16 credits to complete their graduation requirements may be eligible for “per credit tuition” assessment. This form should be submitted at the Student Service Center after students have registered for all courses and credits needed to satisfy degree requirements.
