
Pre-Matriculation Information
Health Information Requirements
The Health Form Booklet must be completed & returned to The Health Center prior to the following due dates.
September admission: September 30th
January admission: February 15th(All completed documents should be mailed to: Emerson College, Center for Health & Wellness: 120 Boylston Street, Boston, MA 02116 or bring the booklet to our office at 216 Tremont Street Room #303 during the Orientation/Registration period. )
Completion of the booklet is a pre-entrance requirement for:
- All students carrying 9 or more undergraduate credits
- All students carrying 6 or more Graduate credits
- All Communication Sciences & Disorders majors, regardless of number of credits
- All students attending Emerson on a student or other visa, regardless of number of credits
- Any part-time degree student electing the College sponsored insurance program and or electing to pay the Health Fee.
Students are expected to meet the College and state health requirements outlined on the Immunization verification page within 30 days of the semester in which they initially enroll.
*Meningococcal Immunization- Legislation was amended in November 2006. Emerson College policy requires all newly entering undergraduate students to submit proof of meningococcal immunization within the last 5 years OR complete the MDPH waiver to meet this requirement. Emerson College Graduate students may elect to receive this immunization; however they are exempt from this requirement.
HEALTH INSURANCE
The Commonwealth of Massachusetts requires all students enrolled at least 3/4 time in a certificate or degree granting program (9 credits for undergraduates or 6 credits for graduates) to document coverage of a qualified Health Insurance Plan. Information regarding the College sponsored insurance plan is mailed separately from Student Financial Services. A copy of the program benefits/premium can be viewed at: www.chickering.com. Students who are enrolled in a qualified insurance program may waive the health insurance fee by submitting the insurance waiver online to the Student Financial Services office within the first 10 days of the semester in which they enroll.
HEALTH FEE
All undergraduate students are assessed the health fee. Graduate students who enroll in the College sponsored insurance program will be assessed the Health Fee. Graduate students with private insurance wishing to receive care at the Center may elect to pay the health fee. The fee covers all office visits to the Center for Health and Wellness and the Counseling Center as well as access to health education programs and materials.
HEALTH WAIVER
All entering students have a Health Waiver designation (HLTH-WVR/ New Student) placed on their registration records until the Health Form Booklet has been reviewed and determined to meet College and Commonwealth of Massachusetts pre-matriculation health requirements. This waiver is valid for the first 30 days of enrollment and allows initial registration, class attendance and student housing assignment. Students requiring additional immunizations to meet compliance will find the HLTH-WVR followed by the date the immunization must be received by in order to meet compliance. Students who fail to meet compliance by the end of the expiration period will have a Health Hold on their account which will prohibit registration and residence in college housing in the subsequent semester until all health/immunization requirements have been met.
