Compliance Forms For Enrolled Student-Athletes

Any enrolled Emerson student wishing to tryout for any of our varsity teams must review and sign the above forms and submit them to the Athletic Department by July 15, 2008. Upon returning to campus for the fall semester, you will be required to sign additional NCAA forms to be eligible to compete.
If you are a new International student-athlete, you will also have to complete the General Amateurism and Eligibility Form. These students will need to provide appropriate visa and/or other government documents.
Please ensure that you have completed the forms thoroughly, as any omissions may result in your inability to practice. No individual may practice or compete for any team if his/her forms are not filed with the compliance office.
Please note that the physical form required by athletics is separate from the general physical forms required by the college. If you have recently had a physical, a doctor can often sign the athletics physical form without an additional appointment. Per NCAA rules, a physical must be completed no more than six months prior to the start of the student-athlete's first day of practice in the traditional season.

Portal to Portal Transportation Release Form
All Emerson College student-athletes are expected to travel to and from away competitions with their teams. Should a student-athlete's parent(s) or legal guardian(s) be in attendance at the competition, the student-athlete may be released by the coach to travel with the parent/guardian. Any student-athlete wishing to do so must complete the Portal to Portal Transportation Release form. This form is also available from the coach.
Transfer Permission to Self-Release
Any Division III student-athlete who wishes to transfer into either into or out of Emerson College and continue to play athletics at Emerson or a subsequent Division III institution must fill out a Permission to Self Release form. the form should be forwarded to the compliance coordinator of the institution the student-athlete wishes to attend. The subsequent institution has a 30 day window of confidentiality to contact the potential transferring student-athlete. Should the student-athlete wish to extend the self-release, a second self-release form must be sent to the subsequent institution. Upon receipt of the second form, the institution to which the student-athlete is interested in attending, must notify the student-athlete's current institution.
